§ 102767 Birth Certificate Name Removal

This law lets a parent who has cancelled a voluntary parentage declaration ask the state to take their name off the child's birth certificate and get a new one.

Key Takeaways

  • •You can request removal of your name after rescinding a voluntary parentage declaration.
  • •You must submit an application to the State Registrar and pay the required fee.
  • •The registrar will issue a new birth certificate if the original record is on file.

Example

A mother signed a voluntary parentage form, later decides she doesn't want to be listed as a parent, and wants her name removed from the birth certificate.

She files an application with the State Registrar, pays the fee, and the registrar creates a new birth certificate that no longer shows her name, as long as the original birth record is on file.
