§ 102200 Vital Records Form Standards

This law says the State Registrar decides what record forms look like and gives them out, and counties have to use only those approved forms.

Key Takeaways

  • •The State Registrar chooses the design and content of all record forms.
  • •Counties must use the exact forms or electronic formats the registrar sets.
  • •No other versions of the forms are allowed.

Example

A county office wants to make its own form for a birth certificate.

Because the law says only the registrar's forms can be used, the county must use the form the registrar provides and cannot create a new one.
