§ 102125 Birth Certificate Requirements

This law says birth, fetal death, and death certificates must be written clearly in black ink and must include every piece of required information; if anything is missing, the certificate isn’t considered complete.

Key Takeaways

  • •Use durable black ink for all birth and death certificates.
  • •Write the information so it can be read easily.
  • •Include every piece of information the form asks for; leaving anything out makes the certificate incomplete.

Example

A nurse writes a newborn's birth certificate in pencil and forgets to fill in the baby's weight.

Because the certificate isn’t in black ink and is missing required info, it isn’t a valid, complete record under this law.
