§ 14218 Voter Name Change Procedure

• •Sign your old surname and your current surname on the same line.
• •Use brackets or another clear mark to show the two names belong to one person.
• •Only needed when your last name changed after you first registered to vote.

Jane Doe gets married and becomes Jane Smith, then goes to the polling place to vote.

When Jane signs the voter list, she writes "Doe (Smith)" or "Doe [Smith]" so the clerk knows it's the same person who registered as Doe.
